As we know, in the first anime season of Kimetsu no Yaiba, Muzan has the appearance of "Michael Jackson", which is a young man in a suit and fedora hat. In this "form", his wife and children call him "Tsukihiko". At the end of the anime as well, around episode 23, Muzan is seen seeing a lower moon in the form of a woman with long hair who wears a black kimono with a flower motif.

In the manga, after the Mugen Train arc, Muzan reappears in the form of a pale-skinned boy who has an allergy to sunlight. In the form of these children, he is named Toshikuni. In Toshikuni's "form", he doesn't look like a demon at all. But when Akaza came to give a report, instantly his nails grew and his eyes suddenly sharpened like demon eyes.

Muzan's next form is his form when he is fighting. In this form, his hair turns white, his hands and feet grow fleshy with sharp mouths and teeth, his legs turn into monster feet with sharp nails, and a type of whip emerges from his very sharp back ribs.

In his final form, Muzan turns into a very large baby as tall as a three-story building. This form is his form when he wants to survive the attacks of Tanjiro and the demon slayers. In this form, he appears to be trying to escape the sunlight, his only weakness.
